Note: Not all forms can be submitted on myApprenticeship – check your form for details.
This form is used to apply to the department to amend the apprenticeship or traineeship training contract. Examples include changes to:
- the start date of the apprenticeship or traineeship
- employment status (e.g. from part-time or school-based to full-time)
- the qualification
- the qualification level (e.g. from Certificate II in Retail Operations to Certificate III in Retail Operations)
- date of birth - where date of birth impacts on legislated age requirements.
This form is used by employers to notify the department of a change of the legal name, legal name and ABN or change of trust and/or trustee name.
This form is to be used by employers who have been declared as a prohibited employer under the Further Education and Training Act 2014, to apply to the department to revoke this decision.
Apprentices and trainees may use this form to give authority to the department to release information to a third party person or organisation.
This form is used to apply to the department to cancel the training contract of an apprenticeship or traineeship where both parties mutually agree.
This form is used by one party of the training contract to apply to the department to cancel the contract where the party believes either themselves or the other party cannot successfully complete the obligations of the training contract.
This form is used by an apprentice or trainee, employer or SRTO to apply to the department to change the mode of delivery of the training plan.
This form is used to notify the department of a change of supervising registered training organisation for one or multiple apprentices and/or trainees.
This form must be completed by the employer, apprentice or trainee and supervising registered training organisation (SRTO) to notify the department when an apprentice or trainee has achieved all competencies required in their training plan and a qualification has been issued by the SRTO. The department will then use this form to issue a completion agreement for the apprenticeship or traineeship.
This form can be used for the completion of electrical apprenticeships, instead of ATF-011. This combined form has two purposes:
- as a completion agreement in the apprenticeship completion process, and
- to apply to the Electrical Safety Office for an electrical work licence (apprentice).
This form can be used to apply to the department for the issue of a completion certificate if the supervising registered training organisation (SRTO) for the apprentice/trainee has stopped operating as a registered training organisation (RTO) before a completion agreement is signed by the parties and the SRTO.
This optional template, when completed and signed, may be used by the parties as evidence of their negotiated and agreed schedule of school studies, work and training for a school-based apprentice or trainee.
ATF-013 Employer resource assessment
The employer resource assessment (ERA) is completed and retained by the supervising registered training organisation to verify that the employer of an apprentice or trainee is able to provide, or arrange to provide, the facilities, range of work, supervision and training required under the training plan.

This form is used to apply to the department to extend the nominal term (end date) of the training contract, therefore extending the apprenticeship or traineeship.
This form is used to apply to the department to extend the probationary period of an apprenticeship or traineeship. The application must be made at least 14 days before the original probationary period expires.
This form is used to notify the department of a minor amendment to the training contact, such as:
- change of apprentice or trainee's name (e.g. through marriage or by deed poll)
- change of a party's address
- change of an employer's trading name
- correction of a typographical error in a party's name or address.
A change of qualification due to the current qualification being superseded, use the ATF-028 Transition of qualification form.
A supervising registered training organisation who is reasonably satisfied an apprentice or trainee has completed their apprenticeship or traineeship, but is unable to obtain one of the party's agreement or signature, can use this form to request the department to investigate the issue.
This form is used to notify the department of a sale of employer's business, dissolution of employer's partnership or a change in employer's legal name.
This form is used by employers to notify the department where the employment of an apprentice or trainee has ceased.
NOTE: This form is NOT required if both parties have agreed to cancel the training contract and have signed a ATF-034 Cancel a registered training contract form.
Permanent transfer of a registered training contract
Please answer the following question according to your current situation to ensure you are provided with the correct form.
I am applying to permanently transfer my training contract from my current employer to a new employer and both employers are in agreement for the transfer to occur.
This form is used by school-based apprentices to notify the department of a discrepancy in the number of days worked that has been calculated by the department. The number of days worked must be accurate to ensure correct allocation of QCE credits.
This form is used to apply to the department to temporarily suspend the training contract for reasons such as seasonal employment, long term illness/injury or maternity/paternity arrangements.
This form is used by either the employer or apprentice/trainee to apply to the department to temporarily suspend the training contract for a period of up to 12 months, where it is reasonably believed that one party cannot perform their obligations under the training contract.
This form is used by the employer to apply to the department to temporarily suspend the training contract for no more than 30 days.
This form is used to notify the department of a temporary transfer of a training contract from one employer to another. It is only necessary when the new employer is taking on all obligations relating to the training contract.
This form is used by apprentices and trainees whose training contracts have been cancelled, and who wish to apply for government funding to continue their training in the same qualification with their training organisation.
Information about the training contract which is required to be signed by the employer and apprentice or trainee at the commencement of an apprenticeship or traineeship.
This form is used by the SUPERVISING REGISTERED TRAINING ORGANISATION ONLY to notify the department of a transition of qualification for one or multiple apprentices and/or trainees, due to an update to the national training package.
Travel and accommodation forms
The fastest way to apply for the travel and accommodation subsidy is via Travel and Accommodation Online.

This form is used by apprentices and trainees to claim travel and accommodation subsidies when attending off-the-job training with their supervising registered training organisations. Visit the Travel and accommodation subsidy page for eligibility requirements.
TRAINING ORGANISATIONS please note: You must ensure all pages of the claim form is provided to the apprentice or trainee as they must read the Conditions of Air Travel prior to signing the claim form.
This form is used by apprentices and trainees to apply to the department for air travel to attend off-the-job training at their supervising registered training organisation (SRTO) only if they either:
- reside in Cape York Peninsula or the Islands of the Torres Strait; or
- are required to travel 1,100km or more (return) to the location of their SRTO.
